I agree to have my pet(s) leashed or kenneled during drop off and pick up. If you do not have a leash one will be provided for you before exiting vehicle with pet.

Health or Medical Problems

Vaccinations: Owners must keep their pets up to date on all vaccinations. If for any reason your pet gets sick in our care you take all liability and responsibility for full financial expenses in seeking medical attention. Occasionally, grooming can expose a hidden medical problem or aggravate a current one. This can occur during or after grooming. All medical expenses for veterinary care will be covered by the pet's owner upon signing this contract/ agreement.

Allergies and Sensitivities:

Your dogs safety and comfort is our primary concern. Please provide us with any allergies sensitivities or preexisting medical conditions so we can avoid aggravating these situations. Please let us know of any prior grooming history you or your dog may not have found satisfactory. We want to avoid the repetition of poor or unpleasant experiences or situations.

Accidents

Although accidents are very rare, there is a risk when dealing with animals. Grooming equipment is sharp, and although we use extreme caution and care in all situations, possible problems could occur including cuts, nicks, scratches, quicking of nails, etc. In most cases this can happen when an animal is wiggling or moving around. Your pet's safety and comfort is our number one priority. In the event an accident does occur, you will be notified of the accident. If we see it is serious, and the owner is not on-site, we will seek immediate veterinary care for your pet.

Matted Coats

Animals with severely matted coats require extra attention. Mats in an animal's coat grow tight, and can ultimately damage and tear the animal's skin, which provides a breeding ground for parasite infestations. We will not cause serious or undue stress to your pet by de-matting. After-effects of mat removal procedures can include itchiness, skin redness, self-inflicted irritations or abrasions and failure of the hair to regrow. In some cases, pets may also exhibit brief behavioral changes. If your pet needs to be shaved to remove matting, by signing below, you acknowledge that you agree to this procedure and any risk. There may be an additional charge for this process.

Completion Time

Every effort will be made to keep our scheduled groomers running smoothly. A typical pets groom can be completed in 2 to 4 hours PER PET. If your pet has behavior issues or skin and coat problems you may be expecting a longer period of waiting time. If you need your pet returned at a certain time please let us know prior to the groom. We are happy to work with you as much as possible, but repeated unexpected visits to check on the readiness of your pet only causes further delays. It makes if extremely difficult to finish your pet if it sees you while still being worked on. Owner must wait for pick up call before returning to pick up pet.

Hard to Handle and Aggressive Pets

Your pets safety, comfort, and will being is our primary concern. We reserve the right to alter, refuse, or charge extra on any groom in the event of staff determining it is in your pets best interest or your pet displays aggressive behavior. A Cut Above Pet Salon has the right to refuse service to customers and their pets at any time for any reason. Customers also understand they assume all responsibility and expense including medical bill, damages & lost wages if my pet bites or injures a person or pet. The customer assumes all responsibility and expenses including but not limited to any damages caused to or by pet.

I have reviewed this Service Contract for accuracy and understand the contents of this contract. I affirm that I am the rightful legal owner of the dog(s) for which services are being rendered. I authorize this signed contract to be valid approval for future grooming services. I understand that pricing is subject to change. This agreement is valid for all pets that I currently own or will own in the future. I have read, signed, and agreed to the above.
